Introduction
This document provides an overview of what you need to do in order to complete the portfolio assessments. In addtion to your written work you will also have to pass two theory exams. This is done through the Active IQ online exam platform (called Cirrus). Your tutor will arrange this once you have agreed a date for the exams.
Case Study – steps 1 and 2 (Food Diary)
Select your client according to the guidance given on page 7
Before issuing the Food Diary you need to ascertain your client’s healthy eating goals (see page 7). You can record the outcomes of a discussion with your client or you could ask them to email you and then print a PDF of this email to show their nutrition-related goals.
Please ask your client to complete a 7 Day Food Diary using the template below (or an equivalent which must contain the same informatioin)
You can then analyse their food diary using this document
Once completed, the Food Diary and your analysis sheet will be be marked by your assessor using this mark scheme. Make sure you read this mark scheme to ensure you cover everything required.

Your PT Toolkit Section 4 contains information about postural analysis. You can also use this template to analyse posture – it should be included in the documentation submitted when you have completed the consultation process.
During the consultation you are required to explain fitness assessment protocols to your client. These should be relevant to the client’s goals. Just remember not every test is suitable or safe for every person.
